First time booking with us?

Please complete the required forms before your pet’s stay.

Forms You Will Need Submitted

Please insure to file the following information in our forms section before you start your booking request to ensure we have all the information we need to complete a booking.:

  • Vaccination records — Dogs must be current on Rabies, DHLP, and Bordetella. Cats must be current on Rabies and FVRCP.
  • Veterinarian contact — Name, clinic, and phone number.
  • Emergency contact — A friend or family member we can reach if we cannot reach you.
  • Feeding details — Brand of food, portion size, schedule, and any food sensitivities.
  • Medications — List of any medications with dosage and timing instructions.
  • Special needs — Any behavioral notes, anxieties, or special care instructions.
  • Hurricane evacuation contact — Name and phone of someone we can reach in case we can’t make contact with you and your pets need to be relocated during a storm.

Grooming notice: Grooming appointments are sometimes booked out a month or more. An employee will reach out via your preferred contact method below to schedule.

Day-of services: Baths and nail trim/dremel may be available the same day, depending on staffing.
